Job Summary
The Social Workers for the Geriatric Psychiatry Community Service (GPCS) team work in a role of case management. All the case managers on GPCS focus on supporting clients residing in the community sector and one long term care home. The Social Workers work in partnership with individuals, families and groups to access services, co-create ideas to enable clients to find effective solutions to problems and improve their quality of life. Social Workers offer a distinctive role in resolving barriers to service utilization and to helping families negotiate the system. The proportion of time spent in various social work functions will vary according to the area of service assigned.
Social Workers use their knowledge of human behaviour and family relationships to provide counselling, education and support to people with mental health illnesses and their circle of care. The role of the Social worker is to work in collaboration with the Geriatric Psychiatrists to actively participate in assessment and interventions.
Key Responsibilities
- Comprehensive psycho social assessment and focused consultations of community dwelling seniors completed within the home setting, institutional setting or outpatient service which address the unique needs of individual clients
- Intervention and/ or referral and linkage consultation with relevant providers or agencies
- Ongoing case management within the mandate of the team serving the client and family
- Provision of individual or group counselling to community-based seniors and / or caregivers
- Telephone/virtual support and consultation as needed
- Developing and sustaining mutually trusting relationships between family members, staff and clients and helping the client and their family members get connected to the residence, hospital or day program, and make appropriate use of the resources offered.
- Counselling clients and families regarding psychosocial issues associated with aging and mental health which may include loss and mourning, cognitive, physical and emotional declines and transitions/ adjustment to change
- Assisting with utilization of resources, referral and coordination within Baycrest and with appropriate agencies and services
- Helping individuals maximize their control over their own lives through participation in and development of their respective groups and community and utilizing a strengths-based approach that affirms capabilities, resourcefulness and resilience of clients and families
- Participation in quality management, evaluation, monitoring and improvement initiatives within social work and assigned teams, programs and/or services.
- Assisting with transitions in the community sector
- Crisis counselling to help connect clients and families to the necessary supports
- Advocacy on behalf of clients and their circle of care
Qualifications
- Masters of Social Work degree
- Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’)
- Experience with individual and family counselling; case management: community resources
- Group work skills, program development planning skills and evaluation of effectiveness
- Comprehensive skills, knowledge in the field of geriatrics
- Strong interpersonal, communication and problem-solving skills
- Interest / experience in the integration of social work care, research and education
- Experience in Mental Health preferred
- Experience with Responsive Behaviours preferred
- Experience working in a healthcare setting or mental healthcare setting preferred
- Basic computer skills – Meditech, Microsoft Word and Excel, RM&R preferred
