Muskoka Algonquin Healthcare (MAHC) is a multi-site organization that includes the Huntsville District Memorial Hospital Site and the South Muskoka Memorial Hospital Site, and together we provide outstanding, integrated care to support people in living their healthiest lives.

OVERVIEW

This role is responsible for assessing post-acute patients, managing CIHI data submissions, and ensuring accurate, timely reporting to support funding and compliance. The position also leads and champions FIM® and MDS completion while collaborating closely with interdisciplinary teams to coordinate patient referrals and rehabilitation planning.

ROLE

• Assess post-acute patients using required data sets within designated timeframes

• Prepare and format data for CIHI submission using approved applications

• Complete electronic data submissions and uploads on the CIHI platform

• Verify data acceptance using CIHI operational reports

• Ensure timely submissions aligned with fiscal quarter and year-end deadlines

• Participate in on-site rounds and rehabilitation rounds as required

• Lead and champion accurate and timely FIM® assessments and compliance

• Support and coordinate appropriate patient referrals to rehabilitation programs

• Collaborate with interdisciplinary teams to identify patient needs and complete rehab applications

• Serve as a key contact for assessments while ensuring data accuracy, funding compliance, and MDS completion

• Other duties as assigned

QUALIFICATIONS

• Registered Practical Nurse registered with the College of Nurses of Ontario (CNO) in good standing
• Current BLS
• As a part of onboarding, it will be a requirement to complete relevant learning pathways through CIHI and other relevant vendor trainings
• Completion of Hemispheres Course desired

ADDITIONAL INFORMATION 

Location: Multi-Site, working on site at both the South Muskoka Memorial Hospital (SMMH) located in Bracebridge and Huntsville District Memorial Hospital (HDMH) located in Huntsville. Applicants must reside within 125km of one of our hospital campuses or be willing to relocate prior to starting. *Exceptions will be considered on a case-by-case basis

Hours of Work:  Current hours of work are Monday - Friday 8:00 am - 4:00 pm. Shifts and hours may change according to departmental requirements per the collective agreement.
