Assess and coordinate health and human services support for patients/clients aged 65 and older
-
Serve patients referred from:
-
South Muskoka Memorial Hospital Emergency Department,
-
In-patients designated as ALC (Alternate Level of Care),
-
Community-dwelling clients in South Muskoka (excluding those in Long Term Care).

Conduct client and family education
-
Provide strategies and interventions to help clients maximize independence
- Liaise with committee groups and provide referrals to other professionals
REQUIRED EDUCATION & CERTIFICATION(S)
- Current registration with the College of Nurses of Ontario in good standing
- BLS (Basic Life Support) certification
- Valid Ontario driver’s license
- Familiarity with ADP (Assistive Devices Program) funding agencies an asset
- MoCA (Montreal Cognitive Assessment) certificate an asset
- Familiarity with grief and loss, government grants, pension benefits, Mental Health Act, and Dementia Training an asset
REQUIRED SKILLS & EXPERIENCE(S)
- Minimum 3 years of experience in the provision of evidence-informed practice in a community and/or hospital health care setting
- Well developed knowledge of the role of community agencies and services, and other professions in the care of patients age 65 years and older
- Knowledge and skills in geriatric assessment and screening
- Experience in working within a multi-disciplinary team and interdisciplinary geriatric assessment

ADDITIONAL INFORMATION
Location: South Muskoka Memorial Hospital (SMMH) located in Bracebridge. Applicants must reside within 125km of one of our hospital campuses or be willing to relocate prior to starting. *Exceptions will be considered on a case-by-case basis
Hours of Work: Shifts will fall Monday to Friday between the hours of 8:00 am and 5:00 pm. Shifts and hours may change according to departmental requirements per the collective agreement.
