ROLE
-
Conduct clinical social work assessments and provide management for patients in the Emergency Department and other assigned areas.

Develop and implement individualized plans of care, including counseling, discharge planning, financial assistance, and referrals.

Provide psychosocial support for patients transitioning from hospital care.

Offer specialized support to patients experiencing substance use concerns and those affected by intimate partner violence (IPV).
-
Maintain accurate and confidential patient records in compliance with institutional and professional standards.
-
Participate in interdisciplinary team meetings, quality assurance programs, and professional development opportunities.
-
Deliver educational sessions to staff, patients, families, and volunteers as needed.
-
Establish and maintain partnerships with hospital and community agencies for enhanced patient support.

Contribute to the development of new mental health programming as needed.
QUALIFICATIONS
- Masters of Social Work (MSW)
- Minimum 2 years’ experience in an interdisciplinary hospital setting
- Minimum 2 years' experience working with mental health population
- Registration with the Ontario College of Social Workers and Social Service Workers
- Certification in addiction treatments and medicine
- Experience with:
- Conducting locus and crisis assessments
- Pediatric mental health population
- Working with crisis programs with OPP
- Working with indigenous populations
- Courses or certifications relevant to mental health population, crisis assessments, trauma, resilience, violence and debriefing an asset

ADDITIONAL INFORMATION
Location: Multi-Site, working on site at both the South Muskoka Memorial Hospital (SMMH) located in Bracebridge and Huntsville District Memorial Hospital (HDMH) located in Huntsville. Applicants must reside within 125km of one of our hospital campuses or be willing to relocate prior to starting. *Exceptions will be considered on a case-by-case basis
Hours of Work: Currently Monday - Friday from 7 am - 3 pm however shifts and hours may vary and change according to departmental requirements per the collective agreement.
