Working together to provide outstanding integrated health care to our communities delivering best patient outcomes with exemplary standards and compassion, Muskoka Algonquin Healthcare (MAHC) is a multi-site organization that includes Huntsville District Memorial Hospital Site and South Muskoka Memorial Hospital Site.
ROLE
As a Health Records Clerk at Muskoka Algonquin Healthcare you will;
- Retrieve/collect discharged records from patient care areas and assemble charts according to predefined chart assembly order; incorporates loose reports (labs, radiology reports, transcribed reports) into charts.
- Process inpatient and outpatient records including filing and identifying documentation deficiencies.
- Receive requests for patient records/reports (scanned, microfilmed, and paper) with chart tracking/sign out and delivers to patient care areas.
- Accurate filing and retrieval of inpatient and outpatient records (requests from patient care areas, month-end coding, audits, other request etc.).
- Separate and distribute transcribed reports and distributing and/or mailing copies to physicians/facilities as requested.
- Identify and correct duplicate Medical Record Number (MRN). Merging and chart files as required.
- Answer incoming calls to the department, responding to requests from authorized parties for patient information.
- Perform release of information duties and provide support to the Health Information Management Professionals by retrieving, copying and mailing/faxing records.
REQUIRED EDUCATION & CERTIFICATION(S)
- Graduated with a secondary school diploma or equivalent.
- Medical Terminology Certificate.
REQUIRED SKILLS & EXPERIENCE(S)
- Excellent communication and interpersonal skills.
- Alphabetical and numerical filing accuracy.
- Basic knowledge of office equipment.
- Excellent organizational skills.
- Ability to prioritize work.
- Ability to interpret instructions and fulfil as requested.
- Excellent computer skills having specific experience with Microsoft Word and Excel.
- Keyboarding skills (60 wpm).
DESIRED EDUCATION, CERTIFICATION(S), SKILLS & EXPERIENCE(S)
- Prior related hospital experience is an asset.
- Experience with healthcare-oriented software programs.
- Experience with ADT is an asset.

ADDITIONAL INFORMATION
Location(s): Multi site, working at both the South Muskoka Memorial Hospital (SMMH) located in Bracebridge and Huntsville District Memorial Hospital (HDMH) located in Huntsville.
Department: Health Information Services.
Hours of Work: Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements per the collective agreement.
Conditions of employment for external hires includes;
- Proof of immunization and vaccination records to the Hospital’s Occupational Health and Safety department.
- Legal entitlement to work in Canada.
- Clean Criminal Record & Judicial Matters Check.
