- Review medication orders for safety and appropriateness, and verify orders in an electronic computerized health information system
- Provide drug information and clinical pharmacy support (e.g. pharmacokinetic monitoring, etc.)
- Responsible for the oversight of drug distribution services in collaboration with other pharmacy staff (including IV medications, TPN, chemotherapy, narcotics etc.)
- Assisting in the investigation and reporting of medication related incidents
- Ensure provision of pharmacy services in compliance with accepted standards and relevant legislation (accreditation, Can. Soc. of Hosp Pharmacists, Food & Drug Act, Regulated Health Professions Act, Public Hosp. Act, NAPRA, USP 797, etc.)
- Effectively communicate with nursing, medical staff, pharmacy team members and management
- Act as immediate supervisor and resource person to pharmacy technicians, students, etc.
- Other duties as assigned
- Graduate of a degree program in Pharmacy from a recognized University (B. Sc Pharm. or PharmD)
- Holds a current license from the Ontario College of Pharmacists
- Member in good standing of the Canadian Society of Hospital Pharmacists an asset
- Three (3) to five (5) years' experience as a Pharmacist working in a hospital pharmacy department an asset
- Management experiences an asset
- Possess sound professional judgement, initiative, innovation, ability to organize, co-ordinate, communicate, discipline, delegate and communicate well with all professional groups within the Hospitals
- Enrolment in the Healthcare of Ontario Pension Plan (HOOPP).
- Collaborative and supportive team environment.
