Working together to provide outstanding integrated health care to our communities delivering the best patient outcomes with exemplary standards and compassion, Muskoka Algonquin Healthcare (MAHC) is a multi-site organization that includes Huntsville District Memorial Hospital Site and South Muskoka Memorial Hospital Site.
ROLE
- Completes individual client assessment with respect to physical status, previous diabetes management and control, understanding of disease process and self-care choices based on the Stages of Change model
- Provides nursing education and follow-up related to diabetes management to clients as appropriate
- Participates in classroom and individual teaching as required
- Initiates appropriate client referrals to community agencies and/or other health care professionals as required
- Collects and reports appropriate program statistics including those for quarterly Ontario Health reports
- Provides nursing information regarding diabetes management to team members and other health care providers
- Plans, implements and evaluates appropriate program evaluation criteria related to nursing standards based on the clinical Practice Guidelines and Standards of Care for Diabetes Education in Canada
- Establishes appropriate linkages within the community, sponsoring agencies to promote diabetes awareness and education
REQUIRED EDUCATION & CERTIFICATION(S)
- Bachelor of Science degree in Nursing (BScN) preferred
- Current registration with the College of Nurses of Ontario (CNO) and certificate of competence with CNO
- Certified Diabetes Educator designation
REQUIRED SKILLS & EXPERIENCE(S)
- Minimum 3-5 years’ experience in diabetes care
- Previous experience in adult education, patient counselling
- A minimum of 3-5 years in an interdisciplinary health care setting
- Proficiency in the use of computers and various applicable software (e.g., PowerPoint, Word, excel, electronic health records)
- Excellent time management skills
- Familiarity with diabetes technology such as blood glucose monitoring systems and insulin pumps
- Effective critical thinking, problem solving, organization and leadership skills

- Insulin Pump training

ADDITIONAL INFORMATION
Location: Multi-Site, working on site at both the South Muskoka Memorial Hospital (SMMH) located in Bracebridge and Huntsville District Memorial Hospital (HDMH) located in Huntsville. Applicants must reside within 125km of one of our hospital campuses or be willing to relocate prior to starting. *Exceptions will be considered on a case-by-case basis
Hours of Work: Must be available to work shifts, including Monday-Friday day shift. Shifts and hours may change according to departmental requirements.
