The Geriatric Emergency Medicine (GEM) Nurse will collaborate with members of allied health team and external partners to ensure high quality, effective and efficient services and care is provided to the aging population. Their focus will be on those frail seniors who are at risk of suffering adverse events, loss of independence and admission to hospital or long-term care. The GEM Nurse will provide expert and targeted assessment and treatment services in the ED; they enhance linkage with hospital and community services and are advocates of the seniors they serve. As well, in their role as geriatrics educators and capacity builders within the ED, the hospital and the community, Geriatric Care Coordinator help to build the system’s capacity to ensure that frail seniors at risk receive the right care in the right place, at the right time.
ROLE
- Screening & Assessment: Implement standardized protocols to assess high-risk seniors in the Emergency Department (ED) and provide specialized assessments for frail elderly patients, including their health, function, emotional status, and home environment.
- Care Coordination & Consultation: Collaborate with healthcare professionals to develop and implement care plans, offer consultations on geriatric health issues, and ensure appropriate community linkages for services like primary care, long-term care, and community supports.
- Geriatric Education & Capacity Building: Serve as a resource and educator for ED staff, hospital teams, and the community on geriatric care, ensuring that frail seniors receive optimal, timely care.
- Quality Improvement & Best Practices: Participate in initiatives to improve senior-friendly hospital care and promote best practices in elder care, including timely discharge planning and implementing the Home First approach.
- Advocacy & Family Support: Assess and support the coping strengths of elderly patients and their families, advocate for the frail elderly, and help maintain independence and prevent unnecessary hospital admissions.
REQUIRED EDUCATION & CERTIFICATION(S)
- Baccalaureate level preparation required.
- Master’s degree in nursing or Gerontology considered an asset.
- Registered nurse in good standing with the College of Nurses of Ontario
- 3-5 years’ Emergency experience
- Additional training or experience in Gerontology is required, and/or suitable candidate commits to complete education plan for specialized Geriatric training
REQUIRED SKILLS & EXPERIENCE(S)
- Working knowledge of Ontario Health and community resources
- Knowledge and skills in functional assessment and the assessment of physical and mental illness of the elderly
- Evidence of strong clinical knowledge and decision making
- Ability to change, innovate, and be flexible in an ever-changing environment
- Demonstrates excellence in interpersonal communication, critical thinking, and health teaching
- Ability to provide support evidence-based interprofessional patient/family centered care

ADDITIONAL INFORMATION
Location: Multi-Site, working at both the South Muskoka Memorial Hospital (SMMH) in Bracebridge and Huntsville District Memorial Hospital (HDMH) located in Huntsville.
Department: Emergency Department/Discharge Planning Team
Hours of Work: Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements and as per the collective agreement.
Conditions of employment for external hires includes;
- Proof of immunization and vaccination records to the Hospital’s Occupational Health and Safety department.
