ROLE
This role will support hospitals in the development and implementation of regional best practices and improve the standardization of practice across NSM hospitals by providing clinical leadership and mentorship for staff in the assessment and management of older adults with cognitive impairment and responsive behaviours.
- Improvement of the quality of care older adults with cognitive impairments and responsive behaviors receive while in hospital
- Capacity building for MAHC health care professionals in respect to assessment and management of older adults with cognitive impairment and responsive behaviors
- Improvement of hospital flow and reducing ALC days in older adults with cognitive impairment and responsive behaviors
- Enhancement of partnerships and service delivery knowledge between acute care and community
- Collaboration and community of practice within the LHIN and SGS relative to the care of older adults with a cognitive impairment and responsive behavior
- Provide patient consultation
- Provide education /training on a regular basis
- Provide mentorship and ‘just in time’ learning
REQUIRED EDUCATION & CERTIFICATION(S)
- Current registration in good standing with the College of Nurses of Ontario
- Current BCLS Certification
- Current ACLS Certification
REQUIRED SKILLS & EXPERIENCE(S)
- Minimum of 4500 (3 years) hours hospital experience.
- Adhere to strict confidentiality rules
- Demonstrated commitment to interdisciplinary care and evidence-informed practice;
- Ability to collaborate with others, mobilize a team and foster successful partnerships;
- Proven decision-making, critical thinking and conflict resolution skills;
- Ability to work independently and take initiative in a high volume, busy work environment requiring flexibility and adaptability
- Proficient with computer programs like Microsoft Office (i.e. Word, Excel, PowerPoint)
DESIRED SKILLS, EXPERIENCE(S) & EDUCATION
- GPA & PIECES training preferred
- Experience in development of care pathways, clinical tools and education programs preferred
- Proven knowledge and appreciation in care of older adults with cognitive impairment and responsive behaviours
- Demonstrated commitment to interdisciplinary care and evidence-informed practice
ADDITIONAL INFORMATION
Location: South Muskoka Memorial Hospital (SMMH) and Huntsville District Memorial Hospital (HDMH)
Department: Inpatient & Rehabilitation Services (Med/Surg/CCC).
Hours of Work: Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements per the collective agreement.
