What we are looking for:

The Nurse Practitioner (NP), Community Health Clinic, will provide comprehensive primary care services for unattached patients over the age of 6 months within our geographic area of the Ontario Health Team. This role involves a strong leadership component with specific focus on innovative care pathways for high risk paediatric and adult patients as well as connections with patients to help deter ED visits.  This role also may include medical assessment, treatment and evaluation, education and health promotion, disease and injury prevention, acute medical treatment, rehabilitation and support for complex medical and frail elderly patients.  The NP will also provide support as needed for other patient types requesting primary care in the community such as palliative patients, paediatrics, and those searching for Cancer screening.   The NP will need advanced knowledge and decision-making skills to provide a comprehensive health focused approach in meeting the needs of this population.

The NP will practice within the interprofessional team model and be responsible for promoting best practices in complex medical, geriatric and palliative care, paediatrics and promote excellence in the nursing care of complex and frail adults through direct patient care and consultation. The NP will work collaboratively with OHT medical lead, primary care physicians, other NPs and community partners caring for patients in the community.  The NP will work closely with the Medical Lead in establishing priorities for this new model and all other programs available and offered by the Ontario Health Team partners.  

 

What you bring to the role:

  • Master’s Degree in a related field or working towards
  • Current registration from the College of Nurses of Ontario as an RN (EC) - Nurse Practitioner-Primary Health Care (PHCNP) or Nurse Practitioner-Primary Care
  • Demonstrated current expertise in primary care and management an asset
  • Membership in the Registered Nurses’ Association of Ontario & Nurse Practitioner Association of Ontario, preferred
  • National certification in other specialities including Gerontology Nursing from the Canadian Nurses Association and paediatrics, preferred
  • Advanced clinical knowledge and implementation of evidenced based practice
  • Superior interpersonal, coaching, and leadership skills
  • Excellent decision making, problem recognition and problem solving skills
  • Demonstrated commitment to continuing professional development
  • A strong desire and capacity to promote professional practice and excellence in evidence-based, patient centered care
  • Demonstrated competency in the following areas: health assessment and diagnosis; health care management and therapeutic intervention; health promotion and prevention of illness, injury and complications in the care of complex medical patients, paediatrics and geriatric patients.
  • Demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the RN(EC) legislated scope of practice and designated medical directives, as appropriate
  • Advanced knowledge, skill and decision making and expertise in the nursing care of primary care clientele
  • Facilitates application of relevant information to make effective judgments and decisions to lead the management of care for older adults
  • Effective communication skills, engages others, and is a reflective listener and learner
  • Evidence of program design, resource development and project management skills
  • Self-directed, works independently and able to achieve outcomes
  • Proven analytical thinking and proactive problem solving and decision making skills
  • Excellent report writing skills
  • Proficient with computer programs including, Word, PowerPoint and Outlook.
  • Working knowledge of the College of Nurses of Ontario standards of practice
  • Demonstrated good attendance and performance record with the ability to maintain these same standards
  • Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

  

Work schedule:

The successful candidate will work 8 hour day and evening shifts, Monday to Friday.

Compensation:

Salary Band C:
$55.39- $69.27 per hour
