What we are looking for:

Reporting to the Patient Care Manager, Medicine, the Access and Flow Medicine nurse will work in collaboration with Emergency Department (ED) and Medicine teams and corporate stakeholders including the Access & Flow ED RN and Manager of Access & Flow/Administrator on Site (AOS), to support flow efficiencies using business intelligence tools/dashboards. The Access and Flow Medicine nurse will collaborate with key team members to actively re-assess admitted inpatients in the ED to actively plan for the most appropriate care and disposition of the patients (i.e. admission, discharge, telemetry, etc.). to support efficient and timely patient admissions and discharges and, resolution of patient flow issues that arise including, but not limited to early discharge, transfers, diagnostic imaging, allied health assessments and interventions, MRP re-assessment and follow-up. They will educate patients and families about unit-based processes, support admission assessments and expedite admission and discharge process for patients on the medical ward.

 

 

What you bring to the role:

1.       Current Certificate of Registration with the College of Nurses

2.       Current BCLS certification required and ACLS certification an asset 

3.       Minimum 3 years of recent acute medicine department experience 

4.       1-2 years Leadership Experience (projects, charge nurse/team leader/supervisor/flow leader) preferred

5.       RNAO membership is preferred

6.       Excellent physical assessment skills; Physical Assessment courses are an asset

7.       Proficient computer skills 

8.       Demonstrated commitment to a patient/family centred care environment

9.       Demonstrated excellent critical thinking and problem solving skills

10.   Demonstrated ability to work effectively within an interprofessional team as well as independently 

11.   Well developed organization, communication, interpersonal and teaching skills

12.   Evidence of ongoing professional development, continuing education and leadership experience

13.   Demonstrated good attendance and performance records with the ability to maintain these same standards

14.   Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization related policies, health and safety legislation and best practices and completing relevant mandatory education as required

 

Work Schedule:

The successful candidate must be available to work 8/12 hour day/evening shifts with flexibility to cover additional shifts as required.  Please note schedule may change due to operational needs.

 

Compensation:

BAND J: 

Hourly:$37.96 - $54.42 

Annually: $74,022 - 106,119
